Conditions for obtaining service
- The head of the household must be under compulsory subscription.
- The son(s) must be handicapped or 70% physically disabled.

Procedures
- The applicant shall submit the old cards and the enclosed documents to the Reception Desk officer.
- The form and the enclosed documents will be verified for completeness.
- If the submitted documents are not complete, the applicant will be asked to update the required documents.
- If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
- The auditor endorses the voucher.
- The applicant pays the prescribed fees at the cashier’s counter.
- Submit the form and the enclosed documents to the Reception Desk officer.
- The Applications Intake officer enters data.
- The auditor receives the application and validates information against it.
- In case of errors, the application is sent back to the Applications Intake officer to double check the entered dat
- In case the application is complete, the Audit and Posting officer will post the application.
- The application is sent electronically to printing department.
- The new card is printed and handed over to the Reception Desk officer.
- The Reception Desk officer then gives the new health card to the applicant.
Required Documents
- For the handicapped son, a medical report from the district committee stating that the son is handicapped.
- For the physically disabled son, a medical report from the district committee stating that the son is 70% physically disabled.
- A copy of the ID card of the son or the Family Book.
- One (1) personal photograph.
